DENIED OR CUT OFF

Was your Medical Assistance or MinnesotaCare denied? Here's what you can do.

You usually have 30 days from the date of your notice to appeal, or up to 90 days if you have a good reason for being late. A denial is often caused by missing proof, and fixing that is faster than an appeal. We help you find out which it is. Call MinnAssist at (866) 655-6606.

First, read the notice

It tells you what was decided, the reason, the date it takes effect, and the deadline to appeal.

Fix it or appeal

If proof was missing, we help you send it in. If you think the decision is wrong, you can appeal.

How to appeal

Use form DHS-0033, or call DHS Appeals at 651-431-3600 or 800-657-3510. For decisions about a private MNsure plan, appeal through MNsure. To keep your coverage while you appeal, appeal before the date the change takes effect.

We are not lawyers. For legal help, contact LawHelpMN (lawhelpmn.org).
